Required Documentation

  • Picture ID (name and address on ID MUST match your bill)
  • Birth certificate of children in household (DOB)
  • Printout of any child support from family court
  • Pay stubs/ Income from the last 30 days
  • Printout from the unemployment office if you draw benefits
  • Award letter from Social Security or Current Bank Statements if Direct Deposited
  • Proof of all income from everyone in the household
  • Expenses: receipts from the last 30 days showing use of income (Utilities, rent, any loans, Car payment, Insurance)

ASSISTANCE IS BASED ON INCOME to DEBT.

If you do not bring in expenses you cannot not be assisted!

Utility Assistance

  • Original Bill for the utilities that you are seeking help
  • Prior bill for utilities (2 most recent bills)
  • Must have breakdown of what is owed on bill
  • Disconnect or delinquent letter is required

Rental Assistance

  • Must have rent/lease agreement documentation
  • Past due rent letter from The Salvation Army filled out by the Landlord
  • W-9 form completed by Landlord company
  • Must be at least 30 days past due or have an eviction notice
  • If you are 15 minutes late for your appointment you will be asked to reschedule.
  • If you fail to make the scheduled time of your appointment you will not be eligible to apply for assistance for 90 days
  • If you do not have all your necessary paperwork you will be asked to reschedule for next available appointment.
  • We only assist once every 12 months starting with the day we helped.
  • We will assist all clients or households a maximum of three (3) times in 5 years.

It is our goal to make use of our and your time wisely. If you cannot make your scheduled appointment please call so that someone else can have your appointment time.

If you miss your scheduled appointment you may not schedule another appointment for 90 days.
