Onsite Services

Each resident will meet with a case manager weekly and will have access to the following onsite services:

Community/Socialization

SHIP (Supportive Housing Innovative Project) will strive to create a home-like, caring, and supportive environment. The recreational specialist, with input from residents, will develop and implement activities such as weekly support groups, community dinners, educational programming, recreation, and special events.

Independent Living Skills

The vocational counselor and recreational specialist will coordinate onsite training for residents. Topics will include financial management, nutrition, and maintaining a home. These workshops will help residents conform to their lease obligations and ensure they will maintain their housing. In addition, the training will provide skills necessary for long-term independence.

Crisis Intervention

In order to minimize the risk of relapse, case managers will intervene as early as possible when a resident appears to be in trouble. When crises do occur, policies and procedures will be in place to handle them. A staff person will be available 24 hours a day.

Vocational/Educational

SHIP (Supportive Housing Initiative Partnership) will provide vocational and educational programs are designed to enable every individual to realize his or her goals and dreams. Residents will be scheduled to meet with a vocational counselor within two weeks of moving into Hampton Creste, who will complete a thorough employment assessment to determine each resident’s short- and long-term career goals, the aim being to match the skills, values, and interests of each resident with a potential occupation. If necessary, residents will be referred to GED classes, vocational training, or college programs. For those residents ready to work, case managers will offer job readiness workshops and help identify subsidized and non-subsidized employment through job development. Once a resident is employed, staff will continue to provide intensive support to ensure job retention and career advancement.

Case Management Activities

The case managers will help residents identify and achieve their goals and teach them how to access resources to address their physical, psychological, and socials needs. Additionally, the case manager will negotiate, advocate, inform, coordinate, and serve as a liaison to other professionals and service programs. These include educational programs, vocational programs, and access to medical providers, entitlement centers, and advocacy groups.

Additional Onsite Supportive Services:

  • Coordinated after-school activities for school-age children
  • Access to subsidized childcare
  • GED courses, job readiness, and other life skill classes
  • Transportation

The Staff

SHIP will be staffed fully and include the following positions to provide a comprehensive and well-rounded program:

  • Program Director (1 FTE)
  • Case Managers/Service Coordinators (2.5 FTE)
  • Residential Aides (2 FTE, 2 PTE)
  • Recreation Specialist (1 FTE)
  • Vocational Counselor (1 FTE)
  • Job Developer (1 PTE)
